Applying Forceps
Plastic Surgery: Breast Implants
Procedure: Their should be a line marking all cuts prior to the medical instruments and the forceps are used. After placing the implant in the breast in the proper place, epinephrine solution 1:10,000 should be injected inside the pectoral muscle; however, to avoid penetration of the chest wall, extra care must be taken. Final adjustment in the placement of the prostheses may be made after placing the patient in a sitting position to assess size, contour and symmetry.
Inframammary: A 3-4 cm incision is placed just above the inframammary crease. inferiorly to the pectoralis fascia, A flap is created. Between the posterior capsule of the breast and the pectoralis fascia a plane is developed (sub glandular approach). A pocket is created by blunt dissection to accommodate the implant. Care is taken to avoid intercostals nerve damage. Prior to the implanted inserted, the meticulous hemostasis must be obtained. If you have a fiber-optic retractor than you may want to use it. This process should be repeated bilaterally. Following the insertion of the implants, the pateint can be placed in a sitting postion to asses the size of the augmentation. The skin may be closed using a running subcuticular closure.
Preparing the pateint: With conscious sedation, anesthesia may be either local or general. The patient is placed "symmetrically" on the table in supine position with arms extended, abducted and secured in position on padded arm-boards at approximately an 80 degrees angle using padded restraints. to examine the breast size and symmetry, the patient will be positioned in a sitting postion during the procedure. (it is possible for breast distortion could appear in the supine postion. It may be a good idea to fluff the pillow on occasion. You should pad the parts that are vulnerable to neurvascular pressure. Using surgical instrument sets, an electrosurgical dispersive pad is applied.
